Various techniques of transpedicular screw insertion and their effect on pull-out strength (a biomechanical study)

Abstract

Transpedicular screws are, at present, one of the most important methods for fixation in spine surgery. Transpedicular screw holes can be prepared using various techniques. Some surgeons would prefer using a drill for preparing the screw-holes while others prefer tapping as well after drilling. On the other hand some may prefer preparing a small hole in the cortex and direct insertion of the screw. Other techniques are using a curette or probe before insertion. Studies upto now failed to demonstrate a definitive advantage for any one of these different techniques. The aim of this study is to prepare screw holes for various types of screws using all these different insertion techniques and compare results of pull-out test. When "drill and tap" group is compared to "without drill" group, the pull-out strength of screws inserted after drilling and tapping is found to be significantly more. Furthermore it is noted that the pull-out strength is more in screws with deeper threads. Also in case a screw is pulled-out reinsertion of a larger diameter screw is found to be more effective than insertion of the same diameter screw with cement.
