Doğuştan kalça çıkığının tedavisinde açık redüksiyon ve innominat osteotomi sonuçlarının değerlendirilmesinde değişik yöntemlerin karşılaştırılması

Öz

Doğuştan kalça çıkığı nedeniyle açık redüksiyon ve Salter innominat osteotomisi yapılan 31 hastanın 39 kalçası Barrett, modifiye Severin ve Trevor'un değerlendirme kriterleri kullanılarak değerlendirildi. Hastaların ortalama ameliyat yaşı 26.8 (17-50) ay, ortalama takip süresi 44 (12-96) aydı. Barrett, Trevor ve modifiye Severin değerlendirme kriterlerine göre elde edilen çok iyi ve iyi sonuçların toplamı sırasıyla %89.7, %87.1, %89.7 idi. Hastalarımızda %17.9 avasküler nekroz saptandı. Bunların %12.8'i grade l, %5.1'i grade III idi. Doğuştan kalça çıkığının tedavisinde farklı yöntemlerin kullanılmasının sonuçlar üzerine etkisi olmadığı ve bu tedavi şeklinin 1.5 yaşından büyük hastalarda seçilebilecek uygun bir yöntem olduğu düşüncesindeyiz.

The evaluation of the results of open reduction and innominate osteotomy via different methods in the treatment of congenital hip dislocation

Abstract

Of 31 patients who underwent open reduction and innominate osteotomy due to congenital hip dislocation, 39 hips were assessed by means of Barrett, modified Severin and Trevor evaluation criteria. The mean age of the patients was 26.8 (17-50) months and the average follow-up period was 44 (12-96) months. The total number of the excellent and good results obtained though Barrett, Trevor and modified Severin's evaluation criteria were 89.7%, 87.1%, 89.7% respectively. We established avascular necrosis of 17.9% in our patients. This include 12.8% grade I and 5.1% grade III avascular necrosis of the femoral head. We are of the opinion that utilization of different methods in the assessment of the results of open reduction and innominate osteotomy performed in the treatment of congenital hip dislocation has on the results and that this type of treatment is a proper method to be chosen in patients over 1.5 years old.
