Comparison of three different implant combination in burst fractures in the thracolumbar junction

Abstract

Unstable burst fractures of the spinal column should be treated with surgical reduction, fixation and sometimes decompression especially to prevent the serious local kyphosis deformity resulting from late vertebral collapse. Today, there is no standard implantation strategy for the different levels and types of vertebral fractures. This study evaluated 89 thoracolumbar functional burst fractures that had been operated between December 1989 and May 1993 in the 1st and 2st Department of Orthopaedics and Traumatology, SSK Ankara Hospital, in order to find out the effective implant combination. The mean follow up was 42.2 months at the last visit with a minimum of two years. The patients were analyzed in three groups. The first group had been treated using only hook-rod combinations and consisted of 28 patients, while the second group of 30 patient had been treated using reverse hook-transpedicular screw construction. In the third group 31 patients had been treated with only transpedicular Schanz screws. Preoperative sagittal index value at the fracture level was 24.7¡ for the first group, 28.7¡ for the second group and 25.1¡ for the third group. The correction rate was 66.8%, 79.3% and 68.4% for the three groups respectively. The sagittal index value at the thoracolumbar junction was corrected to physiological values in 64.3% of the patients in the first group, 70% of the patients in the third group and all of the patients in the second group. The study concluded that in the correction and maintenance of sagittal contours, transpedicular screw reverse hook combinations supplied rigid frames supported by the cross-link plates could be one of the first treatment options.
