Comparison of orthoroentgenography and CT scanography in the measurements of extremity lengths

Abstract

In recent years, developments in diagnosis and treatment of extremity length discrepancies led to the need of a most sensitive, easiest, cheapest and least harmful technique for the measurement of the length of the extremities. That CT has been introduced as a frequently used technique for extremity length measurement and it was shown to be superior to orthoroentgenographic technique in cadaveric studies made CT to be used routinely on this topic. CT and orthoroentgenography had no superiority to each other in terms of precise measurement in the subjects without flexion contracture. But in cases with flexion contracture, inaccurate measurements are obtained with orthoroentgenography due to projectional foreshortening. The extremity length measurement were performed with orthoroentgenography and lateral CT scanogram on 40 cases with limb length discrepancy admitted to our clinics between 1993-1995. Thirty-five subjects without flexion contracture or<20¡ were evaluated as the first group and 5 cases with >20¡ as the second group. In Group 1, no statistically significant difference was found between CT scanography and orthoroentgenography (t=0,072, p>0,05). However, the difference was significant in Group 2 (t=2,764, p<0,05). It was concluded that CT lateral scanography was a more precise technique than orthoroentgenography in the cases with flexion contracture.
