Diagnostic and therapeutic arthroscopy of the wrist: clinical experience
Abstract
Wrist arthroscopy can facilitate early definitive diagnosis of soft tissue, carpal bone cartilage, the triangular fibrocartilage complex and carpal ligaments injuries. Many such injuries can be treated successfully with minimally invasive arthroscopic techniques, reducing the morbidity associated with surgical exposure and permitting earlier return to work. Between November 1991 and May 1996 19 wrist arthroscopies were performed in the Department of Orthopedics and Trauma Surgery, University of Akdeniz, School of Medicine. The indications were evaluation of chronic wrist pain, confirmation of wrist arthrography, aiding to reduction of distal radius fracture, and examination of articular surfaces of the radiocarpal joint for planning further surgery. The standard arthroscopy ports were used such as the portal III, IV and the radial midcarpal portal. One patient had grade III cartilaginous lesions, two had scapholunate ligament damages and two patients had TFCC tears otherwise not diagnose. In 14 of these 19 patients the arthroscopic finding was of major influence on further therapy. We conclude that wrist arthroscopy is a powerful tool in diagnosing intraarticular lesions and in planning further wrist joint treatment.
