The application of interlocking intramedullary nailing in the treatment of diaphyseal fractures of the tibia

Abstract

In this paper, the role of the reamerized - interlocking intramedullary nailing technique in the treatment of complicated diaphyseal fractures of the tibia, which is been widely used all over the world, has been evaluated. 16 patients who had been accepted to our clinic during October 1993 and February 1995, were classified according to the fracture type and 6 of them were found to be group A of the AO classification, 7 as group B and 3 group C. 6 of the patients were Integument Open fractures while the other 10 were Integument Closed fractures. The etiology was traffic accident in 11 of the cases, and "falling down" in the rest. 15 of the patients were male and the other one was female. The mean age of the patients was 36. The patients were operated by the same team and with the same technique within a mean preoperative hospitalization period of 14 days. The RTN nail designed by the AO group was preferred as the osteosynthesis material. All of the fractures healed within a mean period of 5 months and the results were found to be excellent in 11 of the cases, good in 4 cases, and fair in 1 of the cases, according to the Klemm-Borner Scale. Under the light of this limited experience, this technique, which can control the length and the rotation of the extremity, presents as a reliable alternative with a high union and low complication rate in the treatment of diaphyseal fractures of the tibia by avoiding the necessity of external additional support and permitting early mobilization with weight bearing.

Tibia diafiz kırıklarında kilitli indramedüller çivi uygulamalarımız

Öz

Bu çalışmada, tüm dünyada yaygın olarak kullanılan oymalı, kilitli intramedüller çivilerin kompleks tibia diafiz kırıklarının tedavisindeki yeri araştırılmıştır. Ekim 1993 - Şubat 1995 tarihleri arasında kliniğimize müracaat eden ve AO sınıflamasına göre 6'sı Grup A, 7'si Grup B, 3'ü Grup C ve 6 tanesi açık kırık olan toplam 16 hasta bu çalışmaya dahil edilmiştir. Kırık etyolojisi 11 vakada trafik kazası, 5 vakadaysa düşmeydi. Hastaların 15'i erkek, 1'i bayan olup, ortalama yaş 36'dır. Hastalarımız preop, geçen 14 gün sonunda aynı ekip ve teknik uygulanarak ameliyat edilmişlerdir. Çivi olarak, AO grubunun dizayn ettiği RTN çivisı kullanılmıştır. Tedavi edilen tüm kırıklar ortalama 5 ay zarfında kaynamış olup, Klemm-Börner skalasına göre 11 vaka mükemmel, 4 vaka iyi, 1 vaka orta olarak değerlendirilmiştir. Bu kısıtlı vaka sayımızın ışığında: ekstremite boyu, aksı ve rotasyonunun kontrol edildiği bu teknik, yüksek kaynama oranı, düşük komplikasyon riski ile dışardan herhangi bir tespite gerek kalmadan erken mobilizasyon ve yüklenmeye izin vermesi nedeniyle bu bölge kırıklarınm tedavisinde geniş ve güvenli bir seçenek oluşturmaktadır.
