Results of the surgical treatment of acetabular fractures

Abstract

At this presented study, between 1987-1993, clinical and radiographic results of 18 patients with displaced acetabular fractures, that had been treated operatively in our department were reviewed. The results were evaluated in 17 patients (excluding an immediate death); according to the quality of the reduction and, Matta's clinical and radiographic criteria. Perfect and good results were obtained clinically in 11 patients (64.7%), and radiographically in 12 hips (70.5) at the average 33 months follow-up period. In 9 patients, that were further followed-up for two more years (average length were 55 months), 6 improved clinically but 2 showed radiographic regression. The overall results were 74.9% (12 patients) and 62.5% (10 hips). Complications were superficial infection, deep infection and late death, malign hyperthermia and death, avascular necrosis of the femoral head, acetabular collapse, K-wire migration, vertical displacement of the hemipelvis, heterotopic ossification, and iatrogenic nerve lesion. In spite of the severity of the peri and postoperative complications, a relatively good rate of satisfactory clinical long-term results indicates that surgical intervention should be applied in selected cases.
