The role of core decompression in the treatment of avascular necrosis of the femoral head
Abstract
The results of core decompression in the treatment of avascular necrosis of the femoral head (ANFH) have been evaluated. 22 hips of 15 patients, 8 of whom were male and 7 were female with an average age of 36.5 years, were evaluated. The average follow-up was 49 (min.24-max.68) months. The etiology of ANFH was steroid administration in 10 patients, alcohol abuse in 2, sickle cell anemia in 1 and idiopathic in 2 patients. Core decompression without any grafting was performed in all patients. Pre and postoperative radiologic grading was done according to Ficat's Classification and the average progression rate was %40 in Grade II hips and %87.5 in Grade III hips. Modified Merle d'Aubigne's Criteria were used for clinical evaluation and average postoperative score was 15.8. Core decompression has a very clear analgesic effect for a relatively prolonged time despite of the radiologic progression and is also a "time-buying" procedure before total joint replacement in this relatively young patient group.
