Treatment of subtrochanteric femoral fractures due to high-velocity gunshot injury

Abstract

In this study, between 1991 to 1995, sixteen patients who had fracture of subtrochanteric femur due to high-velocity gunshot injury were reviewed. The patients were treated by wound debridement, skeletal traction, delayed primary wound repair, parenteral antibiotics and delayed internal fixation with 95¡ AO condylar blade-plate. Internal fixation was performed on average 16 day after trauma. The patients were followed up for an average of twenty eight months. There were two superficial infections that were controlled by local wound care and antibiotics. There were no deep infections or osteomyelitis. Other complications involved were one loss of fixation with non-union and one malunion. The clinical results were satisfactory in 14 patients (87.5%). We observed that parenteral antibiotics should be administered and that delayed internal fixation should be performed in the treatment of subtrochanteric femoral fractures due to high-velocity gunshot injury.
