The results of surgical treatment of femoral intertrochanteric fractures

Abstract

We retrospectively analyzed the clinical and radiological obtain of 141 femoral intertrochanteric fractures of 139 patients who were followed-up at least one year. Average age was 59.8 years. In 48.2 percent of cases Richards' nail, in 19.8 percent of cases Smith-Petersen's nail and Mc Laughlin's plate, in 17.7 percent of cases Jewett's nail, in 7.8 percent of cases AO condyler plate, in 4.9 percent of cases Steinman's nail and in 1.4 percent of cases cancellous screws were used. The result were found very good in 41.8 percent, good in 31.2 percent, fair in 14.2 percent and poor in 12.8 percent. Among various implants, Richard's compression hip screw gave the best results. In the unstable fractures which medial displacement osteotomy was performed, Jewett's nail fixation was found to be less successful then the Richard's nail fixation.

İntertrokanterik femur kırıklarında cerrahi tedavi sonuçları

Öz

Kliniğimizde 1978-1995 yılları içersinde femur intertokanterik kırık nedeniyle cerrahi olarak tedavi edilen 196 hastadan 1 yıl üstünde takip edilebilen 139 hastanın 141 kırığı klinik ve radyolojik olarak incelendi. Hastaların yaş ortalaması 59.8 idi. Olguların %48.2'sinde Richards çivisi, %19.8'inde Smith-Petersen çivisi ve Mc Laughlin plağı, %17.7'sinde Jewett çivisi. %7.8'inde AO kondiler plağı, %4.9'unda Steinman çivisi ve %1'inde spongiozvida fiksasyonu uygulanmıştır, Sonuçlarımızdan %41.8'i çok iyi, %31.2’si iyi, %14.2'si orta ve % 12.8'i kötü olarak bulundu. En iyi sonuçların Richards kompresyon çivili plağı ile alındığı belirlendi. Medial deplasman osteotomisi uygulanan anstabil kırıklarda; Jewett çivisi uygulamalarının Richards çivisine oranla daha başarısız olduğu gözlendi.
