Open fractures of elbow as a result of the gun-shot injuries

Abstract

Open fractures of elbow as a result of the gun-shot injuries are quite rare and usually accompany to upper extremity injuries. Since anatomic location of the structures has close relation to bone, there is a high risk of neurovascular injuries. Seventy-eight open elbow fractures were treated at our clinic between January 1990 and December 1995. All the cases were male and, the average age was 22.3 (19-37). Of 78 cases, 38 were located in region 1, 27 in region 2, and 13 were in both regions. Twenty-two cases had neurovascular injuries. Of 78 cases, 38 had conservative treatment, 24 had external fixator, 14 had internal fixation, and the remaining 2 had elbow prosthesis. Average follow-up was 38 months (24-72). Patients were allowed to early motion. Patients were evaluated clinically and functionally. Infection, pain, range of motion, instability, angular deformity, loss of muscle strength, and degenerative changes were included in evaluation. Of 78 cases, 14 had excellent results, 39 had good, 17 had moderate, and 8 had poor results. Early diagnoses of neurovascular injuries, rigid internal fixation, resurfacing of joint, early mobilization, and infection prophylaxis are the most desirable requirements for open elbow fractures.
