The results and complications of the high tibial osteotomy which is performed in the knee osteoarthritis with varus deformity

Abstract

We evaluated the results in sixteen patients (twenty knees) who had proximal tibial osteotomy for unicompartmental osteoarthritis. The operations were performed between 1989 and 1995. The mean length of follow-up was 2 years and 4 months (range, 8 months to 61 months). The average age of the patients was 57.7 years (range, 36-68 years). Before operation all the knees were painful while walking. Further, while 3 of the knees (15%) had severe pain, 15 of them (75%) had moderate pain but 2 of them (10%) had no pain at rest. 3 of the patients with one knee involved (15%) were able to walk and stand more than 30 minutes. After operation, 3 of the knees (15%) had no pain at all, but 13 of them (65%) had mild pain, 4 knees (20%) had moderate pain at rest. While 6 of the knees (30%) were able to walk and stand unlimited, 7 of them (35%) more than 30 minutes and 7 of them (35%) between 15-30 minutes were able to walk. All of the patients came, more or less, better condition according to "The Hospital for Special Surgery criteria". The good results which are achieved in patients whom procedure is done early in the course of the disease and with correctly aligned extremities, lasted as much as 5 years. Most of the bad results depended on technical faults and incorrectly chosen implants and beginning post-op early period they needed serial operations. As most of the complications depends on these two main reasons, we had delayed union in two knees, screw protrusion in the joint in one knee, reflex sympathetic dystrophy in one knee, plate breakage in one knee, pin tract infection in one knee and paresthesia in one knee. We had better results in the knees which procedure had been performed early in the course of the disease. Patients suffered from more pain after average 2 years postoperatively. We concluded that complaints in the earlier period usually depend on the technical faults.
