The results of anterior approach in cervical disc herniation

Abstract

Forty-two cases of cervical disc herniation were operated on via anteriorly at our department between January 1991 and January 1996. In neurological examination, radiculopathy was found in 25 and myelopathy in 17 of the cases. Cervical disc herniation was detected in one level in 21, two levels in 19 and three levels in 2 of the cases. Discectomy was performed in 11 levels and discectomy and fusion in 54 levels. Irrespective of the number of the level; excellent results were obtained in 35 percent of cases with myelopathy and 60 percent of cases with radiculopathy alone. In concern with the number of the level; excellent results were obtained in 50 and 17 percent of cases who had one and two levels disc disease with myelopathy and 82 and 46 percent of cases who had one and two levels disc disease with radiculopathy alone respectively. Improved result was obtained in a patient who had three levels disc disease with myelopathy and in a patient who had three levels disc disease with radiculopathy alone. The results of anterior approach are highly satisfactory in cervical disc herniation. However the results are better in one level disc herniation and radiculopathy alone than more levels and myelopathy.

Servikal disk fıtıklanmalarında anterior yaklaşım sonuçları

Öz

Kliniğimizde Ocak 1991- Ocak 1996 tarihleri arasında 42 olgu servikal disk fıtıklanması nedeniyle ante-rior yaklaşımlı tedavi edilmiştir. Nörolojik muayenede 25 olguda radikülopati, 17 olguda miyelopati bulgu-ları saptanmıştır. 21 olguda bir, 19 olguda iki, 2 olguda üç mesafede fıtıklanma bulunmuştur. Mesafelerin %11’ine diskektomi, 54'üne ise diskektomi vefüzyon uygulanmıştır. Mesafe sayışma bakılmaksızın: miyelopatisi olan olguların %35’inde, sadece radikülopatisi olan olguların %60'ında tam iyileşme gözlenmiştir. Mesafe sayısı gözönüne alındığında; miyelopatisi olan olgulardan bir mesafe fıtığı olan %50, iki mesafe fıtığı olan %17 olguda tam iyileşme, üç mesafe fıtığı olan 1 olguda ise kısmi iyileşme gözlenmiş, sadece radikülopatisi olan olgulardan bir mesafe fıtığı olan %82, iki mesafe fıtığı olan %46 olguda tam iyileşme, üç mesafe fıtığı olan 1 olguda ise kısmı iyileşme görülmüştür. Servikal disk fıtıklanmalarında anterior yaklaşımla tedavi yük-sek oranda tatminkar sonuç vermektedir. Ancak bir mesafe fıtığı ve sadece radikülopatisi olan olgularda so-nuçlar, birden fazla mesafede fıtığı ve miyelopatisi olan olgulardan daha iyidir.
