Surgical treatment of neglected congenital dislocation of the hip in children

Abstract

Between 1993 and 1996 a total of 89 hips in 76 patients were operated on for congenital dislocation A total of 40 hips in 36 patients were evaluated. Last follow-up examinations could be performed on 36 patients with 4 bilateral 21 left and 11 right hips. There were 29 female and 7 male patients. Patients age at the time of operation was an average of 46.7 months (range 24-96 mo). All the cases had fully dislocated hips without previous treatment. 40 hips undervent open reduction+proximal femoral osteotomy and shortening+Salter osteotomy. Average folow-up ranged 31 months. Pre-operatif acetabular index averaged 36.6¡ and was 18.2¡ at the last follow-up. Average collo-diaphyseal angle was 138¡ pre operatively and 113¡ post-operative and 124¡ at last follow-up. Center-edge angels could not be measured preoperatively but at last follow-up was 30.8¡. Average femoral shortening was 1.7 cm intraoperatively decreased to 1.2 cm at the last follow-up. 2 of the hips redislocated and 1 was subluxated. 2 patient had superficial, skin infection that resolved with antibiotics and wound dressings. Clinical evaluation revealed good results in 31 hips. Radiologic examination showed very good and good results in 30 hips. A total of 6 patients had avascular necrosis.
