Use of the suture anchors in surgical treatment of anterior shoulder instabilities

Abstract

Anterior shoulder instability has perplexed surgeons since ancient times. More than 100 procedures have been described to treat this disorder. During the last decade, knowledge of the pathomechanics of instability has increased significantly. 35 patients were operated with the diagnosis of glenohumeral instability between the years 1990 and 1996. 20 patients, whose minimum folow-up period were 2 years, were included in this study. 16 patients were re-examined and 4 patients were interviewed over the phone. 17 patients were male and 3 were female. The average age was 24 years. The average age was 22 on the initial dislocation and the average dislocation episode number was 12(range from 3 to 30). All but two patients had traumatic event for the initial dislocation episode. Apprehension sign and the relocation test was positive in all cases. Sulcus sign was positive in 8 patients. 12 patients had diagnostic arthroscopy. 8 had MRI and 6 had both arthroscopy and MRI preoperatively.18 cases had undergone Bankart repair and 2 cases had inferior capsular shifting by using suture anchors. 6 cases had both medial capsular shifting and Bankart repair. We used GII Mitek in 12, GI Mitek in 4, and Mitek-superanchor in 4 cases. The average follow-up period was 42 months( range from24 to 72 months). We did not note any recurrences in any case. The average score according to the Rowe's scale was 92 (range from70 to 100). The score was excellent (score 90 to 100) in 16 cases, good(score 74 to 89) in 2 cases, and moderate(score 51 to 74) in 2 cases. Except the displacement of one GI Mitek suture anchor in one case, we did not note any complications related to the technique. The use of suture anchors simplified both the Bankart reconstructions and the capsular shifting procedures, however careful attention to anchor placement at the junction of articular cartilage and the glenoid neck is necessary to avoid technical failure. Suture anchors are a satisfactory way to reattach capsuloligamentous tissue to bone to restore stability to an unstable shoulder joint.
