The application of open intramedullary fixation in the treatment of pediatric fractures of the shafts of the radius and ulna

Abstract

Treatment of the forearm fractures under the age of 10 years is conservative. In children over 14 years, these fractures should be managed like adult forearm fractures. In adelosans between 10 and 14 years of age, treatment of forearm fractures differ for the specialty of the case. Between 1988 and 1997 , 35 children with the forearm fractures were operated in our department and open intramedullary fixation was performed for all cases. Final followup could be done for 31patients. In this retrospective study, the results of intramedullary pinning of forearm fractures in children between 10 and 14 years of age were evaluated. The advantages of this technique are that removal of the intramedullary rods is a minor procedure and that no stress risers are created following removal, possibly increasing the risk of refracture which can be seen after plate fixation. Flexible pins allow controlled motion which increases the speed for healing. In open technique no image intensifier is needed. Application is easier and complications are few. As we obtained good clinical results, we concluded that intramedullary fixation is a very useful technique for the unstable fractures of shafts of the ulna and radius in children.
