Results of the conservative and surgical treatment of displaced acetabular fractures

Abstract

We treated 24 displaced acetabular fractures conservatively or surgically with an average follow-up of 7 years. Preoperative evaluation of the patients were made by anteroposterior X-ray of the pelvis according to Rowe-Lowel Classification, Matta Clinical Classification Thompson and Epstein Roentgenographic Classification were used for postoperative evaluation. Twelve of the fractures involving the weight-bearing dome or with complicated unstable hip were treated surgically, the other 12 fractures were treated conservatively. Complications included one sciatic nerve injury, two superficial and one deep infection for the surgery group, and three pin tract infection for the conservative group. The overall functional result was also satisfactory in 58 % for the surgical treatment. The results of fractures which could be reduced by traction were satisfactory in 58 %. Open reduction and internal fixation are indicated for the displaced fractures, closed treatment can give satisfactory results in selected patients.
