Effects of polytetrafluoroethylene surgical membrane on adhesion formation after repair of zone II flexor tendon lacerations in the rabbit

Abstract

Adhesions which develop following Zone II flexor tendon repair is a problem still awaiting solution. In this experimental study PTFE SM (Polytetrafluoroethylene Surgical Membrane) use for the primary tendon repair and sheath reconstruction is macroscopically, histopathologically and functionally evaluated in rabbit animal models. FDP tendons of the 3rd and 4th fingers of the both forefoot of 30 New Zealand albino rabbits were bisected at Zone II and repaired. With 8 fingers in either group, only primary tendon repair was performed in group I (control group); additionelly tendon sheath repair was made in group II in group III primary tendon repair and sheath reconstruction with PTFE SM was performed; in group IV primary tendon repair was performed and PTFE SM was placed circumferentially with epitendinous suture about the tendon repair; in group V primary repair was performed and PTFE SM was placed circumferentially with epitendinous suture about the tendon repair and tendon sheath repair was performed. At the end of the 6th postoperative week rabbits were sacrificed and evaluated. Best macroscopic, histologic and functional results were obtained in group II and V. It was concluded PTFE SM is biologically well tolerated agent leading to no foreign body reaction, and PTFE SM was placed circumferentially about the tendon repair, and sheath repair decreases scar tissue and adhesion formation with better tendon nutrition and formation of a smooth surface resulting is decreased friction to the repair area.
