Early results of lengthening of an intramedullary nail in the treatment of leg length discrepancies of the lower limb

Abstract

The prolonged use of the external fixator is the most diffucult aspect of lower extremity lenghtening for the patient to tolerate. To reduce the time during which the external fixator must be in place, we applied lenghtening of the extremity over an intramedullary nail. In our study we applied femoral lenghtening over an intramedullary nail in five patients. Our purpose was to evaluate this method and to compare it with the current well known Ilizarov method of lenghtening. Both the study and control group shared the same criterias as a age, etiolgy, level of difficulty of procedure and amount of lenghtening. Lenghtening over an intramedullary nail reduced the average duration of the external fixation by almost one-third. The radiographic consolidation index was same for both groups. In the study group, the range of motion of the knee was better after treatement As a complication ; in one case, distal locking screws of the nail were broken after fixator removal as loss of lenghtening did not occur, there was no need for a revision. In our control group on the other hand, there were two fixed flexion deformities of the knee despite vigirous physiotherapy and in one patient, fracture of the regenerated bone occured. In the study group the complication rate was much lower when compared with the Ilizarov group.Based on clinical and radiographic criterias there were one excellent, four good results in the study group compared with one excellent, one good and three fair results in the control group. The advantages of lenghtening over an intramedullary nail include a decrease in external fixation time, thus providing a more comfortable postoperative period and protection of the regenerated bone against refracture.
