Effects of medial arthrotomy and lateral retinacular release on the patellar vascularity

Abstract

Patella related problems are the most frequent complications that occurs following total knee arthroplasty. Avascular necrosis of the patella due to medial arthrotomy and / or accompanying lateral retinacular release is thought as an important cause of these complications.In this study, effects of medial arthrotomy and lateral retinacular release on patellar vascularity are investigated by adaptation of these incisions to rabbits. Medial arthrotomy itself or its combination with lateral retinacular release are applied to the right knees of 40 New Zeland rabbits in two equal groups. At the end of the periods of 3 weeks and 2 months both patellae of each rabbit were removed and examined histopathologically. All the patellae which had any kind of surgery , showed aseptic necrosis with varying ratios. There were no differences between the groups which had medial arthrotomy only and the groups which had lateral retinacular release in addition, neither at the 3 rd week nor at the 2nd month. In clinical relevance, the medial arthrotomy alone can disturb the patellar vascularity significantly. Additional lateral retinacular release does not lead to significant alteration of avascular necrosis. Surgical approaches which spares superomedial structures, such as "Subvastus" approach, can be a better way to protect patellar vascularity.
