Diagnosis and treatment of primary tumors of sacrum and ilium

Abstract

Treatment of tumors of sacrum and ilium are one of the difficult problems of musculoskeletal oncology . Seven patients with tumors involving sacrum and ilium between the years 1996-1998 were investigated in this study . There were 3 chordomas, 2 simple bone cysts, and the others were angiosarcoma and giant cell tumor. Cases with chordoma were older than 40 years and their major complaints were back, leg pain and constipation. Average duration of symptoms were 3.7 years (2-6 yrs.) for patients with chordoma. Diagnosis was performed with open biopsy. Lower blood transfusion rates were obtained compared to the literature. This was possibly due to angiography and embolisation prior to surgery. Bone and soft tissue reconstruction must be carefully planned because of the defect occured after sacrectomy.

Sakrum ve ilium primer tümörlerinin tanı ve tedavisi

Öz

Bu çalışmada 1996-1998 yılları arasında sakrum ve iliumda tümör saptanan ve tedavi uygulanan 7 olgu değerlendirildi. Olguların üçü kordoma, ikisi basit kemik kisti olup diğerleri, anjiosarkom ve dev hücreli tümör idi. Kordomalı olgular 40 yaş üzerinde olup şikayetleri uzun süreli bel ve bacak ağrısı ile konstipasyondu. Tanı açık biopsi ile konuldu. Ameliyat öncesi yapılan anjiografi ve embolizasyon kan transfüzyon oranının literatüre göre düşük olmasını sağladı. Sakrektomi sırasında oluşacak boşluk göz önünde bulundurularak yu-muşak doku rekonstriksiyonunun planlanması dikkatle yapılmalıdır.
