Arthroscopic meniscectomy in ACL-deficient knees in a population of desired activity levels of Tegner 3 to 7

Abstract

37 ACL-deficient knees (37 patients) underwent arthroscopic meniscectomy between 1992 and 1995. Having received detailed information on the procedure, these patients either rejected or were not offerred ACL surgery. 23 patients with an average age of 31 and both in 6 knees. Arthroscopic meniscectomy was done for 27 of 30 tears: 16 partial medial, 9 partial lateral, 2 subtotal medial. 4 knees had Grade 3-4 chondral lesions. X-rays revealed minor degenerative changes in 3 knees. Clinical evaluation consisted of a questionnaire, manual laxity tests, Lysholm score and Tegner activity score. Mean follow-up was 27 months (range, 12-58). 21 felt their knees were better, and 2 did not note any change. 18 patients had varying degrees of complaints. Only 5 patients were asymptomatic. Arthroscopic meniscectomy led to higher scores in 19 knees. Overall results according to the Lysholm scale were as follows: 4 excellent, 7 good, 7 fair, and 5 poor. 16 had accepted activity modification prior to surgery and 7 had not? 5/7 returned to preoperative activities. Overall 65% could return to their desired activity levels. The highest activity level possible without any symptoms of instability was Tegner 6 (recreational basketball, handball, tennis and skiing), but this was not a rule. Tegner 7 was possible only at a lower performance. 7 patients (30%) needed or were offerred ACL reconstruction. Tegner 6 seems to be the borderline activity level for conservative or surgical treatment of ACL deficiency. Indications are clear for higher and lower levels, but a conservative approach for level 6 will yield unpredictable results.
