Comparison of plate fixation and intramedullary Küntscher nailing in the treatment of fractures of femur diaphysis in adults

Abstract

The purpose of this study is to evaluate and to discuss the results of femur fractures treated with plate fixation and intramedullary Küntscher nailing. Between July 1991 and February 1997 we operated 75 patients having femur fractures. We applied plate fixation 44 of them (58.7%) and intramedullary Küntscher 31 out of 75 (41.3%) patients. In plate fixation group 34 of patients were (77.3%) male and 10 of were (32.7%) female. In intramedullary Küntscher nailing group, 21 out of 31 patients (67.7%) male and remaing 10 (32.3%) were female. Mean ages were 29 and 33.3 in plate fixation and in intramedullary Küntscher nailing groups, respectively. 93.2% of plate fixation group were closed, 6.8% of were open fractures. On the other hand, these values were 93.5% and 6.5% in the other group, respectively. Middle third was the most common region in plate fixation and intramedullary Küntscher nailing groups a ratio of 89.1% and 48.1% , respectively. Fall from height was the main etiology in plate fixation (50%) and vehicular accident (61.3%) in intramedullary Küntscher nailing. Mean operation and mean hospitalization time in plate fixation and intramedullary nailing were 130 min and 72 min (p=0.010), and 23.5 days and 8.5 days (p=0.000), respectively. According to Thorasen classifications, functional and radiological total results were excellent or good in plate fixation as 83.8% and in intramedullary nailing as 93.1% (p=0.0475). As a result, intramedullary Küntscher nailing is superior to plate fixation considering the short operation time, avoidance of blood transfusion, short time of hospitalization and union time, low rate of complications and better postoperative fonctional results.

Erişkin femur cisim kırıklarının cerrahi tedavisinde plaklı osteosentez ve intramedüller Küntscher çivileme yöntemlerinin karşılaştırılması

Öz

Amacımız, Plaklı osteosentez ve intramedüller Küntscher çivileme yöntemi ile tedavi ettiğimiz erişkin fe-mur cisim kırıklarının sonuçlarını değerlendirmek ve irdelemektir. Haziran 1991-Şubat 1997 arasında cerrahi tedavileri yapılan 75 olgudan 44'üne (% 58.7) plaklı osteosentez (PO) ve 31'ine (% 41.3) intramedullar Küntscher çivileme (İMKÇ) yöntemi uygulandı. PO grubundaki toplam 44 olgunun 34'ü (% 77.3) erkek. 10'u (% 22.7) kadın ve ortalama yaş 29, İMKÇ grubundaki toplam 31 olgunun 21'i (% 67.7) erkek, 10'u (% 32.3) kadın ve ortalama yaş 33.3 idi. Kırık tipi; PO grubunda % 93.2 oranında kapalı ve % 6.8 oranında açık kırık, İMKÇ grubunda % 93.5 oranında kapalı ve % 6.5 oranında açık şeklindeydi. Lokalizasyon olarak PO grubunda % 59.1 ve İMKÇ grubunda % 48.4 ile orta 113 cisim kırıkları ilk sırada idi. Kırık oluşturan nedenler; PO grubunda % 50 yüksekten düşme ve İMKÇ grubunda % 61.3 oranında trafik kazası ile ilk sırayı alıy-ordu. Operasyon süresi PO grubunda ortalama 130 dk. ve İMKÇ grubunda 72 dk. idi (p=0.010). Hastanede kalma süresi; PO grubunda ortalama 23.5 gün ve İMKÇ gruhunda ortalama 8.5 gün idi (p=0.000). Olguların Thorasen sınıflamasına göre fonksiyonel ve radyolojik olarak toplam değerlendirilmesinde; PO grubunda %83.8 ve İMKÇ grubunda %93.1 oranında mükemmel ve iyi sonuç alındı (p=0.0475). Sonuçta; İMKÇ yönteminin, operasyon süresinin kısalığı, kan transfüzyon miktarının azlığı, hastanede kalma süresinin kısalığı, kırık kaynama süresinin kısalığı, komplikasyonlarının azlığı, postoperatif fonksiyonel sonuçlarının başarısı yönünden, PO yöntemine göre daha başarılı olduğu saptandı.
