Surgical anatomy and safe zones of the superficial peroneal branch of the peroneal nerve in the ankle

Abstract

Superficial peroneal branch of the peroneal nerve was evaluated by dissecting in the 28 ankle of 14 cadaver.In these dissections,we have found three distinct patterns of the superficial peroneal nerve.These were consisted of type A(78.6),type B%(14.3),type C(%7.1).Type A was observed at a mean distance 4.5 cm proximal to the ankle joint divided 2 major branches after penetrated crural fascia.A large branch was medial dorsal cutaneous nerve and a smaller that located more laterally was intermediate dorsal cutaneous nerve.Both medial and intermediate dorsal cutaneous nerve were arisen independently in type B.In this type,medial dorsal cutaneous nerve had a similar course to that found type A,while intermediate dorsal cutaneous nerve penetrated the posterior of the crural fascia.And then this branch coursed laterally from anterior of the fibula at the mean distance 1.5 cm above the ankle.Medial dorsal cutaneous and intermediate dorsal cutaneous nerve were arisen independently in type C.Medial dorsal cutaneous nerve was coursed similar to type A,however intermediate dorsal cutaneous nerve penetrated the crural fascia in the anterior of the distal fibula was seen.This nerve was found at the mean distance 2.3 cm to the lateral malleol. According to these results,iatrogenic trauma may be seen especially in type B and C during the ankle arthroscopy or other surgical procedures in the distal fibular region. This study suggest localization of the superfical peroneal nerve is invariable. Anatomic course and variations to determine the safe zones of this nerve should be known in detail.

Ayak bileği bölgesinde peroneal sinirin süperfisyel dalının cerrahi anatomisi ve emniyet bölgeleri

Öz

On dört kadavranın 28 ayak bileği ve ayak bölgesinde peroneal sinirin süperfisyel dalı disseke edilerek incelendi. Yapılan bu disseksiyonda sıiperfisyal peroneal sinirin 3 farklı tipi tespit edildi. Bunların %78.6'sı tip A, %14.3'ü tip B, %7.1'i tip C'den oluşmuştu. Tip A cruris fasyasından çıktıktan sonra ayak bileğine ortalama 4.5 cm uzaklıkta 2 majör dala ayrıldığı görüldü. Büyük olanı medial dorsal kutanöz sinir, daha küçük olanı intermediate dorsal kutanöz sinir idi. Tip B' de ise hem medial hem de intermediate dorsal kutanöz sinir superfisyal sinirden bağımsız olarak çıkmaktaydı. Bu tip de medial dorsal kutanöz sinir Tip A'ya benzer bir traje izlediği, buna karşılık intermediate dorsal kutanöz sinir krus fasyasının posteriorundan çıktığı tespit edildi. Daha sonra bu dal fibulanın önünden ayak bileğinin ortalama l.5 cm üzerinden laterale doğru bir traje izlediği saptandı. Tip C'de medial dorsal kutanöz sinir ve imermediate dorsal kutanöz sinir süperfisyel peroneal sinirden ayrı olarak çıkmaktaydı. Burada da medial dorsal kutanöz sinir Tip A'ya benzer olmasına karşılık intermediate dorsal kutanöz sinir fibula distalinin anteriorunda krus fasyasını delip çıkmakta olduğu görüldü. Bu sinirin dış malleole olan uzaklığı ortalama 2.3 cm olarak bulundu. Bu sonuçlara göre latrojenik travma özellikle Tip B'de ve Tip C de intermediate dorsal kutanöz sinirde görülmekte olup, distal fibula bölgesinde ayak bileği artroskopisi veya diğer cerrahi işlemler esnasında görülebilir. Bu çalışma yüzeyel peroneal sinirin yerinin sabit olmadığını göstermektedir. Bu sinirin emniyet bölgesinin belirlenmesi için mevcut anaromik trajeler ve varyasyonlar detaylı bilinmelidir.
