Isolated avulsion of the subscapularis tendon insertion (a case report)

Abstract

Isolated rupture of the tendon of the subscapularis muscle has been reported and is an infrequent and usually post-traumatic injury. Here we present the case of a 57-year-old male with a far less common injury, that is, isolated avulsion of the humeral insertion of the subscapularis muscle secondary to minor trauma and without anterior dislocation of the shoulder. The lesion was missed at the initial examination and anteroposterior radiograph and later diagnosed at follow up and confirmed by MRI and CT. Surgical treatment was performed through a deltopectoral approach and transosseous sutures were used for immediate strong repair. The patient had almost full and painless active motion of the left shoulder at four months. Isolated subscapularis tendon tear or avulsion of its insertion in the absence of dislocation is possible even after a minor trauma and it should be actively sought in the presence of anterior shoulder pain and marked limitation of motion.

Subskapular kas tendonunun yapışma yerinin izole avulsiyonu (Olgu sunumu)

Öz

Subskapular kas tendonunun izole rüptürü bildirilmiştir ancak seyrektir ve sıklıkla travma sonrasında karşılaşılan bir yaralanmadır. Bu yazımızda, 57 yaşında bir erkek hastada, minör bir travmaya bağlı ve omuzun öne dislokasyonu ile beraber olmadığı için çok daha nadir olan, subskapular kasın humerusa yapışma yerinin izole avulsiyonunu sunmaktayız. Lezyon ilk fizik muayenede ve Ön-arka grafide gözden kaçmış, daha sonraki kontrollerde MR ve BT ile tanıya varılabilmiştir. Olgumuzda deltopektoral yaklaşım ile cerrahi tedavi uygulanmış ve erken dönemde kuvvetli bir onarım sağlayabilmek için kemik içi dikiş tekniği uygulanmıştır. Ameliyat sonrası dördüncü ayda tama yakın ve ağrısız omuz hareketleri sağlanmıştır. Omuz çıkığı olmaksızın, subskapular kas tendonunun izole rüpturü veya yapışma yerinin izole avulsiyonu minör bir travma sonrasında bile gelişebilir. Omuz önünde ağrı, ileri derece hareket kısıtlılığı ile başvuran olgularda düşünülmeli ve dikkatlice araştırılmalıdır.
