Measurement of ACL length by MRI

Abstract

Bone-patellar tendon-bone and hamstring tendon autografts are the most commonly used grafts in anterior cruciate ligament (ACL) reconstruction. Hamstring tendons are becoming more popular in recent years. With the new fixation methods like Endobutton, it is possible to use hamstring tendons more economically in ACL reconstruction. At this stage we must know the minimum graft length that we need. Minimum graft length is, 1.5 cm for femoral and tibial tunnels plus the length of the graft in the joint. Based on this knowledge in an attempt to evaluate the mean ACL length in our population, 50 young adults (age range 20 and 35) underwent MRI examination for measurement of the length of their ACL. Mean ACL length was found as 34.02 mm (SD: 2.37) in these 50 adults. Our results indicated that at least 6.4 cm graft was necessary in ACL reconstruction including femoral and tibal tunnels and, the graft length in joint. When it is considered that hamstring tendons are generally recommended to be used in 4 folds in ACL reconstruction, the minimum harvested hamstring tendon length must be 25.6 cm. This length is important to decide whether to use semitendinosus tendon alone or with the combination of gracilis tendon in ACL reconstruction.
