Secondary hemiarthroplasty results for displaced proximal humerus fractures

Abstract

Proximal humerus fractures ,which need surgical treatment, have always been subject of discussion. Open reduction + internal fixation and prosthetic replacement are alternatives of treatment. We studied 4 patients, which had been treated surgically because of displaced proximal humeral fracture. Open reduction and minimal osteosynthesis had been performed in other centers and the patients referred to our hospital because of complications of internal fixation. Secondary hemiarthroplasty operations have been done 19 weeks (12-26 weeks) after the trauma.The patients were 61.5 (58-68) years old.The average follow-up period was 30 months(range 9-61months). In one case infection occured, and in another case aseptic loosening made us remove the prosthesis.We had reached very good result in one case and limited satisfactory result in another case according to Neer criteria. Secondary surgery for failed primary treatment is technically difficult, and the results are inferior to those reported for acute humeral head replacement

Humerus üst uç deplase kırıklarında sekonder hemiartroplasti sonuçları (4 olgu sunumu)

Öz

Cerrahı gerektiren humerus üst uç kınklarının tedavisinde açık repozisyon + internal fiksasyon ile hemiartroplasti arasındaki seçim her zaman tanışma konusu olmuştur. 1994-1996 yılları arasında başka hastanelerde açık repozisyon+inrternal fiksasyon uygulanmış ve komplikasyonlar sonucu kliniğimizde sekonder hemiartroplasti girişimi yapılan 4 hasta bu çalışmada değerlendirilmiştir. Sekonder hemiartoplasti uygulanan hastalarımızın ameliyat olana kadar geçen süreleri ortalama 19 haftadır (12-26 hafta). Hepsinde postoperatif standart üç faz fizyoterapi programı uygulanmıştır. Hastalarımızın ortalama rakip sureleri 30 ay (9-61 ay) olup, değerlendirmede Neer kriterleri kullanılmıştır. Sekonder hemiartroplasti yapılan 4 olgudan birinde gelişen infeksiyon nedeniyle, bir diğerinde tüberkülüm minus avasküler nekrozu ve gevşeme nedeniyle protez çıkarılmıştır. Diğer 2 olgudan birinde çok iyi sonuç, diğerinde kısıtlı tatmin edici sonuç alınmıştır. Humerus üst uç parçalı kırıklarında (Neer 3 veya Neer 4) primer açık repozisyon ve minimal osteosentez sonrası sekonder hemiartroplasti uygulamasının cerrahi teknik olarak daha zor ve fonksiyonel sonuç açısından ise daha kötü olduğu sonucuna varılmıştır.
