Anatomic division and safe areas of common peroneal nerve around the fibular head

Abstract

Twenty eight knees of the 14 cadevers were dissected to determine course of the common peroneal nerve from its origin to its termination.Particular dissection was performed to defining: 1. its relationship to fibular head, 2. its branches relationship to fibular head, 3. constituted the average angle between long axis of fibula and its branches, 4. safe areas in the proximal fibula during the surgical interventions. The fibers of the common peroneal nerve were formed three major nerve divisions (anterior recurrent,deep and superficial). The average distance from the fibular head to origin of the common peroneal nerve was 9,9 cm. In the one case, anterior recurrent branch of the common peroneal nerve was coursed just on the fibular head ,in the other case it was coursed above the 2.2 cm the fibular head.It was seen that the angle of anterior recurrent branch to the longitudinal axis of the fibula was 59,82°(59,82±1,46°) and the angle of deep peroneal branch to the longitudinal axis of the fibula was 31,57° (31,57±0,86°). The results of our present study suggest that safe areas for osteotomy or other surgical interventions in the proximal fibula are located up to 1,6 cm.

Fibula başı etrafında nervus peroneus communis’in anatomik dağılımı ve emniyet bölgeleri

Öz

Ondört kadavranın 28 dizinde nervus peroneus communis orijininden sonlandığı kadar trajesini tespit etmek için disseke edildi. Bu sinirin: 1 fibula başı ile olan ilişkişi, 2. dallarının fibula başı ile olan ilişkisi, 3. fibulanın uzun aksı ile onun dulları arasında oluşan ortalama açılar, 4. cerrahi girişim esnasında proksimal fibula'da emniyetli bölgeleri belirlemek için dikkatli bir disseksiyon yapıldı. Nervus peroneus communis'in fibrilleri 3 ana dal oluşturmaktaydı (anterior recurrent, derin ve yüzeyel dallar). Fibula başından nervus peroneus communis'in çıktığı yere ortalama uzaklığı 9,9 cm idi. Bir vakada anterior recurrent dal fibula basının tam üzerinden geçerken, diğer bir vakada 2,2 cm üzerinden geçmekteydi. Anierior reccurrent dalın fibula'nın uzun aksı ile yaptığı açının 59,82° (59.82 ± 1.46°) ve derin peroneal sinir ile yaptığı açının 31,57° (31,57±0,86°) olduğu görüldü. Mevcut çalışmamızın sonuçları proksimal fibula’da osteotomi veya diğer cerrahi girişimler için emniyetli bölgelerin 1,6 cm'ye kadar yerleşim gösteren alanlar olduğunu göstermektedir.
