Congenital indifference to pain and neuropathic arthritis
Abstract
Functional and anatomic integrity of the central and peripheral nervous system is the major prerequiste for maintaining perfect musculoskeletal system function. Sensorial functions and proprioception should be intact for all biomechanical properties. Severe degenerative and destructive joint changes can be caused by lack of protection afforded by the sense of pain. Here we present seven year old twin boys who were diagnosed to have congenital indifference to pain (HSAN type IV). Clinical and radiologic findings and initial diagnosis were supported and confirmed by detailed history and investigation. Accompanying degenerative joint changes, approach for treatment and prognosis are outlined.
