Is the Singh index reliable for the classification of osteoporosis

Is the Singh index reliable for the classification of osteoporosis

Abstract

In 1970 Singh et al have defined the Singh Index in order to use it in the diagnosis and classification of osteoporosis. In recent years, as technology devolop and bone mineral dansitometry was invented, reliability of Singh Index has begun to be discussed. In this study our aim is to search the reliability of Singh Index which shows high interobserver and intraobserver differantiations. Roentgenograms of hips of randomizedly chosen 40 patients were assessed by 10 people by 1 week interval. Results were considered according to Singh Index and a simplified modification of it (Singh 5 and 6=normal, 3 and 4=mild osteoporosis 1 and 2=severe osteoporosis). The ten people were consisting of 5 residents and 5 orthopaedic surgeons. Results were studied statistically for interobserver and intraobserver correlations, and for the correlation between residents and surgeons. Considering the Singh Index, intraobserver correlation was 37%±10 and the kappa value was 0.18±0.11. This correlation is poor. Considering the simplified Singh Index correlation was 59%±5 and the kappa value was 0.26±0.12. So, if the simplified Singh Index is applied, intraobserver correlation rate raises statistically from poor to moderate. Considering Singh Index interobserver correlation was 30%±4, and the kappa value was 0.11±0.03. This shows poor interobserver correlation. Considering simplified Singh Index correlation was 54%±4 and the kappa value was 0.21±0.05. By using the simplified Singh Index interobserver correlation was raised from poor to moderate. Between the residents and surgeons groups no statistically signification difference was found. As a result, we think that, in the diagnosis and classification of osteoporosis, it is better to use simplified Singh Index, which has statistically less interobserver and intraobserver difference.
