Treatment of distal tibial fractures by biological plating

Abstract

The purpose of our study is to determine the efficiency of biological plating for the treatment of distal tibial fractures which were described as complicated fractures when treated by traditional surgical methods. Twelve distal tibial fractures of twelve patients were plated according to biological plating principles and they were prospectively followed averagely for twenty months (range 12 months – 40 months). The mean age of our cases was forty two years (range 21 years – 66 years). The fractures of our cases were classified as four A1, three A2, one B1, one C1 and three C2 according to Muller classification system. Eleven DCP and one T plates were used for internal fixation. All the fractures are healed without secondary operation.No patient had any evidence of infection. One case had ten degrees of varus deformity and ankle motion of two cases were limited about 10 per cent. One case had 1.5 cm of tibial shortening. The results were classified as excellent for eight cases (66 per cent), good for three cases (25 per cent) and fair for one case(9 per cent) according to John and Wruhs criteria. As a result, we conclude that biological plating can be accepted as an alternative and effective surgical method to the traditional surgical methods for the treatment of distal tibial fractures .
