Mallet fracture treatment with extension block technique

Abstract

Mallet fracture treatment usually needs surgery. Reduction of the bone fragment with two percutaneus K-wire is an alternative treatment to open reduction and internal fixation. We evaluated our clinical results of this method. Eight patients were treated with this technique. Blount trauma was the etiology of mallet fracture in all patients. Surgical treatment was performed under regional anaesthesia. Bone fragment was pushed distally with a K-wire under image intensifier control. Extension block was provided with same K-wire. Distal phalan was reducted under image intensifier control and distal interphalangeal joint was fixed with another longitudinally placed K-wire. Despite good ROM one of our patient had an arhtralgia. One patient presented 10 degrees extension lag with good functional result. There was no recurrence and all other patients presented good functional result.
