Mallet fracture treatment with extension block technique

Abstract

Mallet fracture treatment usually needs surgery. Reduction of the bone fragment with two percutaneus K-wire is an alternative treatment to open reduction and internal fixation. We evaluated our clinical results of this method. Eight patients were treated with this technique. Blount trauma was the etiology of mallet fracture in all patients. Surgical treatment was performed under regional anaesthesia. Bone fragment was pushed distally with a K-wire under image intensifier control. Extension block was provided with same K-wire. Distal phalan was reducted under image intensifier control and distal interphalangeal joint was fixed with another longitudinally placed K-wire. Despite good ROM one of our patient had an arhtralgia. One patient presented 10 degrees extension lag with good functional result. There was no recurrence and all other patients presented good functional result.

Çekiç parmak kırığının ekstansiyon blok yöntemi ile tedavisi

Abstract

Kemik parça içeren çekiç parmak deformitelerinin tedavisi genel olarak cerrahi olarak yapılmaktadır. Perkütan iki adet k-teli ile parçanın redaksiyonu ve pozisyonun korunması yöntemi açık redaksiyon internal fiksasyon yöntemine alternatif olabilecek bir yöntemdir. Bu alternatif yöntemin kliniğimizdeki sonuçları değerlendirildi. Kliniğimizde 8 hastada bu yöntem kullanılmıştır. Hastaların hepsinde çekiç parmak deformitesi dorsalde kemik parça ile birlikte olup deformite künt travma ile oluşmuştu. Hastalar lokal anestezi altında manipulasyona alındı. Skopi yardımı ile önce kemik parça K-teli yardımıyla distale itildi ve orada aynı K-teli ile ekstansiyon blok sağlandı. Daha sonra yine skopi yardımıyla distal falanks redükte edildi işlem tamamlandı. Distal falanksın pozisyonu K-telinin longtudinal olarak yerleşimi ile sağlandı. Hastalarımızın birinde iyi hareket ve pozisyona rağmen artralji mevcut idi. Bir hastamızda fonksiyonel sonuç iyi olmakla birlikle 10 derece kadar ekstansiyon yetmezliği mevcuttu. Diğer hastalarımızın hiç birinde deformite tekrarı yoktu ve sonuçları iyi olarak değerlendirildi.
