Açık el yaralanmalarında antibiyotik kemoprofilaksisinin yeri

Öz

Açık el yaralanmaları nedeni ile hastanemize acil olarak başvuran hastalarda, başvuru anındaki kontaminasyonu belirlemek ve antibiyotik profilaksisinin enfeksiyon oranı üzerindeki etkisini ortaya koymak amacı ile prospektif randomize bir çalışma yapılmıştır. Beraberinde tendon, kemik, damar, sinir yaralanması olan veya olmayan toplam 100 vakada çift kör randomize bir çalışma gerçekleştirilmiştir. A grubunu oluşturan 50 hastaya antibiyotik verilmemiş, B grubundaki 50 hastaya ise verilmiştir. A grubunda ezilme yaralanması olan 2 vakada enfeksiyon gelişmiştir. Bu 3 vakada enfeksiyon kalıcı olmayıp, antibiyotik verilmesini takiben ortadan kalkmıştır. B grubunda kemik, tendon ve nörovasküler yapıların yaralandığı parmakta ezilme yaralanması olan bir vaka ve el bileği sevivesinde radial arter ve tendon kesişi mevcut olan diğer bir vaka olmak üzere toplam 2 vakada enfeksiyon gelişmiştir. İlk vakada 3 hafta, 2. vakada 5 gün süreli antibiyotik kullanımı ile enfeksiyon gerilemiştir. Enfeksiyon oranı A grubunda %6, B grubunda %4 olarak gerçekleşmiştir. Enfeksiyon insidansı açısından aradaki fark istatistiksel olarak anlamlı bulunmamıştır. Yara enfeksiyonunu önlemede yara temizliği ve gerekli debridmanların yapılması temel prensiplerdir. Bu çalışmanın sonucunda, profilaktik antibiyotik kullanımının tam debridman yapılamayan ezilme tarzı yaralanmalar ve derin yapıların kontamine olduğu vakalarla sınırlı olması gerektiği düşünülmüştür. Basit deri, izole tendon, damar ve sinir kesisinin bulunduğu, hemen onarım uyguladığımız hastalarda profilaktik antibiyotik kullanı-mının gerekli olmadığı sonucuna varılmıştır. Müdahale öncesi sürüntü kültürünün yapılmasının, daha sonraki dönemde enfeksiyon olasılığının tahmininde veya muhtemel patojenin saptanmasında değerli olmadığı gözlenmiştir.

Role of antimicrobial chemoprophylaxis in open hand injuries

Abstract

A prospective study undertaken to investigate the bacterial contamination at presentation of open hand injuries and the effect of prophylactic antibiotic treatment on the infection rate is reported. A total of 100 patients with traumatic wounds of the hand with or without underlying lesions of bone, tendon, vessel or nerve were randomized to a double-blind trial: Group A, 50 cases were treated without antibiotic administration. Group B, 50 cases were treated with antibiotic administration. In group A, wound infection was observed in 3 crush injury. Of all 3 cases, wound infection was not severe and relevant signs subsided after antibiotic administration. In group B, wound infection occurred in two patients; one presented with dirty, crush finger injury leading into exposition of bone, tendon, neurovascular bundle and amputation of the distal phalanx and another patient presented with the sharp laceration of radial artery and tendons. These patients treated with 3 weeks and 5 days of antibiotic administration respectively. The infection rate was calculated as 6% in group A and 4% in group B. The difference in infection rate between the two groups was not statistically significant. Cleansing of the wound and prompt debridement of all necrotic tissues is critical to prevent the wound infection. As a result of this study we think that, prophylactic administration of antibiotics should be considered only if the adequate debridement of devitalized tissue is not possible due to extensive crush injury or gross contamination of deeper stuructures is present. We conclude that prophylactic antibiotics are unnecessary adjuvant in the treatment of simple lacerations of the hand and when rapid primary repair of the isolated tendon, artery and nerve injury is achieved. Preoperative wound cultures were observed as invalid in predicting the risk of infection and the nature of the likely pathogen.
