Role of antimicrobial chemoprophylaxis in open hand injuries

Abstract

A prospective study undertaken to investigate the bacterial contamination at presentation of open hand injuries and the effect of prophylactic antibiotic treatment on the infection rate is reported. A total of 100 patients with traumatic wounds of the hand with or without underlying lesions of bone, tendon, vessel or nerve were randomized to a double-blind trial: Group A, 50 cases were treated without antibiotic administration. Group B, 50 cases were treated with antibiotic administration. In group A, wound infection was observed in 3 crush injury. Of all 3 cases, wound infection was not severe and relevant signs subsided after antibiotic administration. In group B, wound infection occurred in two patients; one presented with dirty, crush finger injury leading into exposition of bone, tendon, neurovascular bundle and amputation of the distal phalanx and another patient presented with the sharp laceration of radial artery and tendons. These patients treated with 3 weeks and 5 days of antibiotic administration respectively. The infection rate was calculated as 6% in group A and 4% in group B. The difference in infection rate between the two groups was not statistically significant. Cleansing of the wound and prompt debridement of all necrotic tissues is critical to prevent the wound infection. As a result of this study we think that, prophylactic administration of antibiotics should be considered only if the adequate debridement of devitalized tissue is not possible due to extensive crush injury or gross contamination of deeper stuructures is present. We conclude that prophylactic antibiotics are unnecessary adjuvant in the treatment of simple lacerations of the hand and when rapid primary repair of the isolated tendon, artery and nerve injury is achieved. Preoperative wound cultures were observed as invalid in predicting the risk of infection and the nature of the likely pathogen.
