The results of reverse digital artery island flap for fingertip injuries

Abstract

Objectives: To evaluate long-term results of reconstruction with reverse digital artery island flap in patients with fingertip injuries. Methods: The study included 10 patients (15 digits) who completed follow-up examinations after repair of soft tissue losses from fingertip injuries using reverse digital artery island flap. Only arteries were included in the flaps. Mean follow-up was 17 months (range 12-36 months). Final sensory evaluations were made using the Semmes-Weinstein monofilament test. Results: Flap failure was encountered in two digits. The Semmes-Weinstein monofilament test showed normal sensation (2.83) in one flap, diminished light touch (3.61) in eight flaps, diminished protective sensation (4.31) in three flaps, and loss of protective sensation (4.56) in one flap. All digits were normal, without any limitations of movement or contractures. Conclusion: Reverse digital artery island flap seems to be an appropriate material of choice for fingertip injuries with its ease of performance and reliability at a single session and protection of the length of the digit by providing soft tissue coverage with glabrous skin.

Parmak ucu yaralanmalarında kullandığımız ters akımlı dijital arter ada flebi ile sonuçlarımız

Öz

Amaç: Parmak ucu yaralanması nedeniyle ters akımlı dijital arter ada flebi uygulanan hastalarda uzun dönem sonuçları değerlendirmek. Çalışma planı: Parmak ucu yaralanması sonucunda ortaya çıkan defektleri kapatmak amacıyla ters akımlı dijital arter ada flebi uygulanan ve son kontrolü yapılan 10 hastanın 15 parmağı değerlendirildi. Flep içine yalnızca dijital arter alındı. Ortalama 17 aylık (dağılım 12-36 ay) takip süresi sonunda Semmes-Weinstein monofilament testi ile flep üzerinde duyu değerlendirmesi yapıldı. Sonuçlar: İki parmakta flep kaybı meydana geldi. Semmes-Weinstein monofilament testi ile flep üzerinde yapılan duyu değerlendirmesinde, fleplerin birinde normal dokunma duyusu (2.83), sekizinde azalmış dokunma duyusu (3.61), üçünde azalmış koruyucu duyu (4.31) ve birinde koruyucu duyu kaybı (4.56) elde edildi. Parmakların hiçbirinde hareket kısıtlılığı ve kontraktür gelişmedi. Çıkarımlar: Ters akımlı dijital arter ada flebi, uygulaması nispeten kolay ve güvenilir olması, tek seanslı ameliyat gerektirmesi, kılsız derisi olan iyi bir yumuşak doku desteği ile parmak uzunluğunu koruması nedeniyle parmak ucu yaralanmalarında sıklıkla uygulanabilecek bir fleptir.
