Radiologic comparison of crossed K-wires and three K-wires configurations in the treatment of displaced supracondylar humerus fractures

Abstract

Objectives: The aim of this study was to radiologically compare the stability of crossed K-wires and 3 K-wires configurations in the treatment of pediatric supracondylar fractures of the humerus, in regard to loss of correction of Baumann and carrying angles. Methods: Patients who sought emergency treatment for Gartland type III supracondylar humerus fractures were randomly assigned to two treatment groups. Both groups underwent closed reduction and osteosynthesis under general anesthesia. Osteosynthesis was performed using two crossed K-wires in the first group (n=17, 11 boys, 6 girls; mean age 6.1 years) and three K-wires in the latter (n=18, 12 boys, 6 girls; mean age 7.6 years). In all patients, the Baumann and carrying angles were radiologically evaluated both in the intact and involved sides intraoperatively and during the last follow-up examination. Loss of correction in these two angles were calculated. Results: No significant differences were found between the two groups in regard to the Baumann and carrying angles. However, mean value for loss of correction was significantly better in the second group (p=0.003 for the Baumann angle, and p=0.028 for the carrying angle). Conclusion: Compared to the classic crossed K-wires, use of 3 K-wires seems to provide a more stable osteosynthesis in the treatment of displaced supracondylar humerus fractures, without any contribution to morbidity.

Deplase humerus suprakondiler kırıklarının tedavisinde çapraz Kirschner teli ve üç tel konfigürasyonlarının radyolojik olarak karşılaştırılması

Öz

Amaç: Pediatrik suprakondiler humerus kırıklarının osteosentezinde kullanılan çapraz K-teli ve üç K-teli konfigürasyonları, erken postoperatif ve geç postoperatif Baumann ve taşıma açılarında ölçülen korreksiyon kaybı açısından karşılaştırıldı. Çalışma planı: Kliniğimizin acil birimine 1999 yılının ikinci yarısında başvuran Gartland tip III suprakondiler humerus kırıklı çocuklar rastgele seçimle iki tedavi grubuna ayrıldı. İlk gruba (n=17, 11 erkek, 6 kız; ort. yaş 6.1) genel anestezi altında kapalı repozisyon, iki çapraz K-teli ile osteosentez; ikinci gruba (n=18, 12 erkek, 6 kız; ort. yaş 7.6) genel anestezi altında kapalı repozisyon, biri deplasman tarafından olmak üzere üç K-teli ile osteosentez yapıldı. Tüm hastalarda sağlam taraf ve intraoperatif ve son kontrol sırasında kırık tarafında Baumann ve taşıma açıları ölçüldü; bu iki açıdaki korreksiyon kaybı radyolojik olarak hesaplandı. Sonuçlar: Her iki grupta intraoperatif ve son kontrollerdeki Baumann ve taşıma açıları arasında anlamlı farklılık bulunamadı. Ancak korreksiyon kaybı, üç K-teli grubunun lehine, anlamlı fark gösterdi (Baumann açısı için p=0.003, taşıma açısı için p=0.028). Çıkarımlar: Deplase, suprakondiler humerus kırıklarının kapalı perkütan osteosentezi için 3 K-teli konfigürasyonu, çapraz K-teli konfigürasyonuna göre daha stabil bir osteosentez sağlamakta ve ek morbidite getirmemektedir.
