Comparison of diagnostic methods in staging subacromial impingement syndrome
Abstract
Objectives: We investigated the role of diagnostic modalities in staging subacromial impingement syndrome.
Methods: Thirty-nine patients (24 females, 15 males; mean age 54 years; range 36-72 years) with stage 2 and 3 subacromial impingement syndrome, who sought treatment for shoulder pain and limited range of motion were investigated. Staging was made using radiographs, ultrasonography (US) and arthrography. Findings obtained by imaging modalities could be compared with intraoperative findings in 17 patients (43.6%). In cases in which there were no indications for surgery, a comparison with arthrography was made.
Results: Sensitivity, negative predictive value, and false negative rates of arthrography were found as 100%, 100%, and 0%, respectively. On the other hand, sensitivity and negative predictive value for US were 83% and 66%, respectively, with a reasonably high false negative rate of 33%. Sensitivity of US decreased to 63% in patients in whom surgery could not be performed despite an arthrographic diagnosis of complete tear of the rotator cuff.
Conclusion: Arthrography is superior to US in the diagnosis of complete tears of the rotator cuff.
