Early results of cemented total knee arthroplasty

Abstract

Objectives: To evaluate short term results of primary total knee arthroplasty. Methods: Forty-eight knees of 38 patients (35 females, 3 males) were retrospectively evaluated. Patellar resurfacing was performed in 17 knees of 16 patients. Clinical evaluation was made using the Hospital for Special Surgery (HSS) knee scores and patellar scores described by Feller. Preoperative radiographic investigation included varus-valgus deformities, bone quality, osteophytes, and bone defects. Postoperatively, all knees were evaluated using the Knee Society total knee arthroplasty roentgenographic evaluation and scoring system. The mean follow-up was 19.3 months (range 5-40 months). Results: Preoperative and postoperative mean knee scores were 55.9 (range 21-81) and 84.3 (range 30-98), and patellar scores were 13.1 (range 4-27) and 25.1 (range 11-30), respectively. Preoperatively, knee scores were poor in 27 knees (56.2%); excellent, good, fair, and poor results were obtained in 30 (62.5%), 12 (25%), 3 (6.2%), and 3 knees (6.2%), respectively. A tibiofemoral alignment of 5-10° valgus was achieved in 17 knees (35.4%), while 22 knees (45.8%) had 0-4° valgus and nine knees (18.8%) had varying angles. The mean flexions before and after surgery were 102.7° (range 80-140°) and 109.1° (range 0-120°), respectively. No radiolucent lines were detected in 19 knees (39.5%), whereas radiolucencies measured less than 4 mm in 27 knees (56.3%) and 5 to 9 mm in two knees (4.2%). Complications and infections were seen in 16.8% and 6.3%, respectively, being peroneal nerve palsy (n=1), supracondylar femoral fracture (n=1), fibrous ankylosis (n=1), and deep vein thrombosis (n=2). Conclusion: Our short-term results of total knee arthroplasty showed no differences between patients with or without patellar resurfacing.
