The role of arthroscopy in the treatment of tibial plateau fractures

Abstract

Objectives: To evaluate the role of arthroscopy in the treatment of the tibial plateau fractures and to assess our mid-term results. Methods: The study included 49 knees of 48 patients who underwent arthroscopically assisted surgical treatment for tibial plateau fractures. There were 14 females and 34 males with ages ranging from 15 to 68 years (mean 40 years). According to the Schatzker classification, five patients had type I, 18 patients had type II, 13 patients had type III, seven patients had type IV, four patients had type V, and two patients had type VI fractures. Twenty-five patients had accompanying meniscus ruptures, eight of which were treated with meniscal repair. Functional and radiologic results were evaluated according to the Rasmussen criteria and Resnic and Niwoyama criteria, respectively. The mean follow-up was 38 months (range 6-88 months). Results: Functional assessment of the patients were excellent in 17 cases, good in 26 cases, fair in four cases, and poor in two cases. Radiological evaluation showed excellent, good, fair, and poor results in 17, 21, 7, and 4 patients, respectively. Overall, clinical results were satisfactory in 88% and radiologic results in 78%. Complications encountered included deep infection in one patient, correction loss in two patients, and arthrofibrosis in one patient. Conclusion: Arthroscopically assisted surgical treatment of tibial plateau fractures has several advantages. An anatomic reduction of the articular surface of the tibial plateau can be achieved and accompanying lesions such as meniscus ruptures can be managed, with more satisfactory functional results due to early range of motion, and lower mortality rates compared with conventional methods and open surgery.
