Comparison of transosseous suture and suture anchors in the fixation of rotator cuff tears: a biomechanical experimental study

Comparison of transosseous suture and suture anchors in the fixation of rotator cuff tears: a biomechanical experimental study

Abstract

Objectives: This study was designed to compare the primary fixation strength of transosseous sutures and suture anchors in rotator cuff repair. Methods: Thirty-two sheep shoulders were divided into four homogeneously equal groups. In each group, infraspinatus tendons were cut from their insertions and reattached with four diverse techniques, which included a single Mason-Allen stitch (group 1), double Mason-Allen stitches (group 2), double Mason-Allen stitches knotted on the lateral cortex followed by augmentation with a mini metal plate (group 3), and two Corkscrew suture anchors (Arthrex, Germany) (group 4). All specimens were tested for fixation strengths with a Material Testing System machine. Statistical analyses were made using one-way ANOVA and LSD post hoc tests. Results: The mean failure load values were 160.31±34.59 N, 199.36±11.73 N, 182.35±19.19 N, and 108.32±15.98 N in groups 1, 2, 3, and 4, respectively (ANOVA, F=24.63, p<0.001). It was found that group 4 was significantly weaker than the other groups (p<0.001), that the strength of the transosseous suture improved with double sutures (group 1 vs group 2 p=0.002), and that augmentation of the lateral cortex with a metal plate did not improve strength (group 1 vs group 3, p=0.07). Conclusion: We concluded that rotator cuff fixation with only suture anchors cannot provide a reliable strength and that fixation must include multiple sutures if only transoosseous sutures are to be used.
