Neonatal septic arthritis

Abstract

Objectives: This study was designed to evaluate the clinical and radiographic features of neonatal septic arthritis, to investigate the factors facilitating its occurrence, and to determine the causative microorganisms. Methods: Fourteen patients (9 boys, 5 girls) with a diagnosis of neonatal septic arthritis were treated and monitored during a six-year period. All the patients were below two months of age. White blood count, Gram-staining and culture results of the synovial fluid from the affected joint were obtained and C-reactive protein and erythrocyte sedimentation rate were measured. Histories included premature birth (n=4), umbilical catheterization or venous cut down (n=3), sepsis (n=3), perinatal asphyxia (n=2), and difficult labour (n=1). Results: Blood and joint fluid aspirate cultures were positive in seven and eight patients, respectively. Staphylococcus aureus was the most commonly encountered organism. One patient died from sepsis. Deformation and septic dislocation of the femoral head were detected in one patient. Conclusion: Early diagnosis and treatment of neonatal septic arthritis are essential to protect children from impending life-long disabilities
