Results of surgical treatment in distal radius fractures
Abstract
Objectives: To evaluate the results of surgical treatment for distal radius fractures and to determine important factors for the prognosis.
Methods: A total of 40 patients (29 males, 11 females; mean age 41 years; range 13 to 70 years) underwent surgery for distal radius fractures. Prior to decision for surgery, the fractures were assessed with regard to instability. Classification of fractures was made according to AO/ASIF and Frykman systems, and clinical outcomes were assessed by the Sarmiento modification of Gartland and Werley system (Demerit). The mean follow-up was 26 months (range 3 to 37 months).
Results: According to the Sarmiento modification of Gartland and Werley, the results were excellent in 14 patients (35%), good in 16 patients (40%), fair in six patients (15%), and poor in four patients (10%). The most common complication was reflex sympathetic dystrophy in five cases (12.5%), which was ascribed to occurrence of fractures at high ages, poor bone quality, inappropriate adherence of the patients to the rehabilitation programme, delay in surgical treatment, and most importantly, application of distraction with external fixation, and to prolonged immobilization.
Conclusion: Due to our special attention to obtain anatomic restoration, a high percentage of excellent-good results was possible. The type of the fracture, the patient’s age and quality of bone, the timing of surgery, and achievement of anatomic restoration during operation are important factors for a successful outcome in patients with distal radius fractures.
