Scaphotrapeziotrapezoid arthrodesis in the treatment of advanced-stage Kienbock’s disease

Abstract

Objectives: We evaluated the results of treatment of Kienböck’s disease by scaphotrapeziotrapezoid (STT) arthrodesis and lunate excisional arthroplasty with a coiled soft tissue replacement. Methods: Nineteen patients (18 males, 1 female; mean age 23 years; range 20 to 55 years) with stage II or III Kienböck’s disease underwent treatment with SST arthrodesis and lunate excisional arthroplasty with a coiled soft tissue replacement. Fifteen (79%) and four patients (21%) had right and left-sided wrist involvement, respectively. The severity of the disease and clinical results were assessed according to the criteria by Lichtman et al. Radiologically, carpal height and postoperative osteoarthritic changes were examined. The mean follow-up was 30.8 months (range 12 to 87 months). Results: Moderate to severe pain observed preoperatively in all patients disappeared in eight patients. Moderate pain observed in five patients in the early postoperative period completely resolved at the end of the follow-up. Six patients continued to have both early and late postoperative pain. Grip strength increased markedly in 14 patients, remained unchanged in three patients, and decreased in two patients. The results were satisfactory in 13 patients (68.4%), while six patients (31.6%) were regarded as failure. Osteoarthritic changes at the radioscaphoid joint were observed in four patients, two of whom underwent subsequent wrist arthrodesis. Conclusion: Scaphotrapeziotrapezoid arthrodesis maintains carpal height, with markedly reduced pain, improved grip strength, and preserved functional wrist mobility.
