Öz

Amaç: Bu çalışmada varus gonartrozu nedeniyle yüksek tibial osteotomi uyguladığımız olgular retrospektif olarak incelendi. Çalışma planı: Varus gonartrozu nedeniyle yüksek tibial osteotomi uygulanan 54 hastanın 57 dizi değerlendirildi. Hastaların 51’i kadın, üçü erkekti. Ortalama yaş 58.8 (dağılım 43-70) idi. Otuz dört dize Maquet tipi, 23 dize Coventry tipi osteotomi yapıldı. Tespit şekli olarak 34 dizde eksternal tespit, 20 dizde Giebel plağı, 2 dizde staple ve bir dizde alçı uygulandı. Ortalama takip süresi 4.5 yıl (dağılım 6 ay-11 yıl) idi. Sonuçlar: Eksternal tespitlerin çıkartılma süresi ortalama 3 ay (dağılım 2-11 ay) idi. Komplikasyon olarak sekiz dizde çivi yolu enfeksiyonu, iki dizde geçici sinir felci ve iki dizde nonunion görüldü. “The Hospital for Special Surgery” diz değerlendirme kriterlerine göre 45 dizde (%79) sonuçlar çok iyi ve iyi şeklindeydi. Çıkarımlar: Varus gonartrozunda yüksek tibial osteotomi, diz protezini erteleyen ve zaman kazandıran, semptomları rahatlatan ve osteoartritin ilerlemesini geciktiren başarılı bir tedavi yöntemidir.

High tibial osteotomy in the treatment of varus gonarthrosis

Abstract

Objectives: We retrospectively evaluated our patients who were treated with high tibial osteotomy for varus gonarthrosis. Methods: A total of 54 patients (57 knees) with varus gonarthrosis were treated by high tibial osteotomy. There were 51 women, and 3 men (mean age 58.8 years, range 43 to 70 years). The types of osteotomies were Maquet in 34, and Coventry in 23 knees. Fixation was performed using external fixators (34 knees; 33 monoplane, 1 Ilizarov), Giebel plates (n=20), staples (n=2), and cast (n=1). The mean follow-up was 4.5 years (range 6 months to 11 years). Results: The mean duration for external fixation was three months (range 2 to 11 months). Complications included pin tract infections (n=8), transient peroneal nerve palsy (n=2), and nonunion (n=2). The results were excellent or good in 45 knees (79%) according to the knee rating system of The Hospital for Special Surgery. Conclusion: High tibial osteotomy was found to be a successful procedure in avoiding replacement arthroplasty, in relieving symptoms, and delaying the progression of osteoarthritis in patients with varus gonarthrosis.
