High tibial osteotomy in the treatment of varus gonarthrosis

Abstract

Objectives: We retrospectively evaluated our patients who were treated with high tibial osteotomy for varus gonarthrosis. Methods: A total of 54 patients (57 knees) with varus gonarthrosis were treated by high tibial osteotomy. There were 51 women, and 3 men (mean age 58.8 years, range 43 to 70 years). The types of osteotomies were Maquet in 34, and Coventry in 23 knees. Fixation was performed using external fixators (34 knees; 33 monoplane, 1 Ilizarov), Giebel plates (n=20), staples (n=2), and cast (n=1). The mean follow-up was 4.5 years (range 6 months to 11 years). Results: The mean duration for external fixation was three months (range 2 to 11 months). Complications included pin tract infections (n=8), transient peroneal nerve palsy (n=2), and nonunion (n=2). The results were excellent or good in 45 knees (79%) according to the knee rating system of The Hospital for Special Surgery. Conclusion: High tibial osteotomy was found to be a successful procedure in avoiding replacement arthroplasty, in relieving symptoms, and delaying the progression of osteoarthritis in patients with varus gonarthrosis.
