Short-term results of proximal oblique crescentic osteotomy in hallux valgus
Abstract
Objectives: We evaluated the short-term results of a combination of distal soft tissue procedure and a modification of proximal crescentic osteotomy, namely, proximal oblique crescentic osteotomy (POCO) in moderate to severe hallux valgus. Methods: Twenty-five male patients (mean age 21 years; range 19 to 26 years) with moderate to severe hallux valgus and incongruent joint were treated by the distal soft tissue procedure and POCO. Objective evaluations included the hallux valgus angle, the intermetatarsal angle, shortening of the first metatarsal bone, and angulation at the osteotomy site on weight-bearing anteroposterior and lateral radiographs of the foot obtained before surgery and for final examinations. Subjective evaluation was made with the use of a patient questionnaire whose overall scores ranged from 6 (completely satisfied) to 18 (completely dissatisfied). The mean follow-up period was 44 weeks (range 28 to 52 weeks). Results: The mean corrections in the hallux valgus angle and the intermetatarsal angle were 22.1° and 10.8°, respectively. Shortening of the first metatarsal bone (mean 3 mm) occurred in five patients and dorsiflexion deformity of the metatarsal head in one patient, which did not lead to transfer metatarsalgia. Nine patients expressed complete satisfaction (36%), followed by seven and six patients with scores of 7 and 8, respectively. The least satisfaction score was 12 in one patient. Conclusion: The results suggest that metatarsus primus varus, which is the main deformity in moderate to severe hallux valgus, can be corrected by POCO. This technique may be preferable in terms of ease and low complication rate.

Öz
Amaç: Orta ve ileri derece halluks valguslu olgularda kombine distal yumuşak doku prosedürünün ve proksimal kresentik osteotominin modifiye edilmiş bir şekli olan proksimal oblik kresentik osteotominin (POKO) kısa dönem sonuçları değerlendirildi. Çalışma planı: Uygunsuz eklemli orta ve ileri derece halluks valgus deformitesi olan 25 erkek hastaya (ort. yaş 21; dağılım 19-26) distal yumuşak doku prosedürü ve POKO uygulandı. Objektif değerlendirmede, ameliyat öncesinde ve son kontrollerde ayak basarken çekilen ön-arka ve yan grafilerdeki halluks valgus açısı, intermetatarsal açı, birinci metatars boyunda kısalma ve osteotomi hattında açılanma dikkate alındı. Subjektif değerlendirme için hastaların hoşnut kalma düzeyleri bir anket aracılığıyla belirlendi. Yanıtlar, tümüyle hoşnut bulunma (6 puan) ile tümüyle hoşnutsuz olma (18 puan) aralığında değerlendirildi. Hastalar cerrahi müdahale sonrasında ortalama 44 hafta (dağılım 28-52 hafta) izlendi. Sonuçlar: Halluks valgus açısında ortalama 22.1°, intermetatarsal açıda ortalama 10.8° düzelme elde edildi. Metatars boyunda ortalama 3 mm kısalma (n=5) veya dorsifleksiyon deformitesi (n=1) olan hastaların hiçbirinde aktarım metatarsaljisi gelişmedi. Subjektif değerlendirmede tamamen hoşnut grubuna dokuz hasta (%36) girerken, yedi hasta 7, altı hasta 8 puan aldı. En ileri hoşnutsuzluk belirten bir hastanın puanı 12 idi. Çıkarımlar: Orta ve ileri derece halluks valgusta ana deformite olan metatarsus primus varus, POKO ile istenen derecede düzeltilebilmektedir. Erken dönem sonuçlar POKO’nun kolay ve komplikasyonu az bir yöntem olduğunu göstermiştir.
