Treatment of femoral fractures associated with fibrous dysplasia: a case report

Abstract

Fibrous dysplasia is a rare condition in which bone tissue is replaced by fibro-osseous lesions. A sixteen-year-old male patient with fibrous dysplasia underwent plate fixation for subtrochanteric fracture of the proximal femur. Eighteen months postoperatively, progressive femoral deformity and refracture occurred. Upon failure in three-month conservative treatment, the plate was removed and intramedullary fixation was performed. Control radiographs at the end of 18 months after intramedullary fixation showed complete fracture healing. This case emphasizes that intramedullary fixation should be the first choice to treat femoral fractures and to prevent refractures in patients with fibrous dysplasia.

Fibröz displazide femur kırıklarının tedavisi: Olgu sunumu

Abstract

Fibröz displazi, kemik dokunun yerini fibröz dokunun aldığı fibroosseöz bir lezyondur. On altı yaşında fibröz displazili bir erkek hastada femur 1/3 proksimal diafiz kırığına uygulanan plaklı fiksasyondan 18 ay sonra deformitede ilerleme ve tekrarlayan kırık belirlendi. Üç ay süreyle uygulanan konservatif tedavinin başarısız olması üzerine, plak çıkarılarak intramedüller fiksasyon uygulandı. İntramedüller çivilemeden 18 ay sonra yapılan kontrol grafilerinde ve fizik muayenede kırığın iyileştiği gözlendi. Fibröz displazide, özellikle femur kırığı tedavisinde, intramedüller fiksasyonun, tekrarlayan kırık oluşumunu önlemede ilk seçenek olması gereği vurgulandı.
