The effectiveness of arthroscopic debridement and lavage treatment in osteoarthritis of the hip: preliminary results

Abstract

Objectives: We evaluated the early results of arthroscopic debridement and lavage procedure in osteoarthritis of the hip. Methods: The hips of 13 patients (7 women, 6 men; mean age 52 years; range 29 to 74 years) were treated by arthroscopic debridement and lavage in lateral position under general anesthesia. Ten patients had primary coxarthrosis; it was due to acetabular dysplasia in two patients, and to avascular necrosis of the femoral head in one patient. The patients were radiographically and arthroscopically evaluated according to the Kellgren-Lawrence grading system and the Tippett’s chondropathy classification, respectively. Preoperative and postoperative assessments of the hips were made according to the Harris hip scoring system. The mean follow-up was 11.2 months (range 5 to 22 months). Results: Harris hip scores increased in eight patients (%62) postoperatively, namely, in only one patient out of six with a radiographically high grade of osteoarthritis (grade 4), and in all seven patients with grade 2 or 3 disease. No complications occurred. Significant negative correlations were found between radiographic and arthroscopic grades and the clinical improvement (p=0.001). However, patients age and preoperative Harris hip score were not correlated with the clinical result. Conclusion: Arthroscopic debridement and lavage provides significant clinical improvement during short-term follow-up of osteoarthritic hips with radiographic grades of 3 or less.

Kalça osteoartrozunda artroskopik debridman ve eklem yıkanması tedavisinin etkinliği: Erken sonuçlar

Abstract

Amaç: Kalça osteoartritinde, kalça artroskopisi ile uygulanan debridman ve eklem yıkanması işleminin erken klinik sonuçları değerlendirildi. Çalışma planı: On üç hastanın (7 kadın, 6 erkek; ort. yaş 52; dağılım 29-74) kalçasına genel anestezi altında ve hasta yan yatar pozisyonda artroskopik debridman ve eklem yıkanması uygulandı. On hastada primer osteoartrit vardı. Osteoartrit bir hastada femur başı avasküler nekrozuna, iki hastada ise asetabular displaziye bağlıydı. Hastalar radyografik olarak Kellgren-Lawrence osteoartrit evreleme sistemine, artroskopik olarak Tippett kondropati sınıflandırmasına göre değerlendirildi. Ameliyat öncesi ve sonrasındaki klinik değerlendirilmeler Harris kalça skoru ile yapıldı. Ortalama izlem süresi 11.2 ay (dağılım 5-22 ay) idi. Sonuçlar: Sekiz hastada (%62) Harris kalça skorunda artış saptandı. Kalça skorları, radyografik evresisi 4 olan altı hastanın yalnızca birinde, evre 2 ve 3 olan yedi hastada ise çeşitli oranlarda artış gösterdi. Hiçbir hastada komplikasyon gelişmedi. Hem radyografik hem de artroskopik evreler ile ameliyat sonrası klinik düzelme arasında anlamlı bir ters korelasyon saptandı (p=0.001). Hastanın yaşı ve ameliyat öncesi Harris skorunun klinik iyileşme üzerine etkili olmadığı görüldü. Çıkarımlar: Radyografik olarak evre 3 ve öncesi dönemdeki kalça artrozunda uygulanan artroskopik debridman ve eklem yıkanması, ameliyat sonrası erken dönemde anlamlı derecede semptomatik iyileşme sağlamaktadır.
