The treatment of severe scoliotic curves

Abstract

The progressive curves over 100 degrees is commonly seen if the early onset progressive curves could not be treated effectively. Cardiorespiratory failure, back pain, neurological complications and the apparence of bad cosmesis are commonly seen in progressive curves. The complications in the treatment of the progressive curves are high. Correction should be obtained by halofemoral traction preoperatively slowlyand gradually with careful neurologic monitoring, then Harrington instrumentation and posterir fusian should be done without further correction. If the correction is obtained by Harrington instrumentation alone intraoperative spinal cord monitoring or wakeup test should be done. In thes article 17 scoliotic cases, already over 100 degrees which were treated surgically in the department of Orthopedic and Traumatologic Surgery of Ankara University Medical faculty between 1978-1982 are presented. Infection was seen in 2 patients and pseudoarthrosis in 3 patients. Our complication rate was %31.25.
