THE CONTROL OF INFECTION IN COMPOUND FRACTURES, AND THE RESULTS OF ANTIBIOTIC THERAPY AND PROPHYLAXY

Abstract

We have reviewed 570 patients with compound fractures at Gülhane Military Medical Academy and also at Haydarpaşa Military Hospital. We have researched the etioiogical factors that causs compound fractured and their distrubution to the bones and sex. The types of compound fractures were cassified according to the Gustilo classification. Those compound fractures were brought to our clinics within first 12 hours and others who had their first aid treatments at the other clinics also being brought to our clinics later than 12 hours were compared. We obtained far better results in our patients who were brought to our clinics within first 12 hours than the others. We came to the following conclusion Kefzol and Gentamycin are the best antibiotics in compound fracture prophylaxy. As tor peniciline, it was clearly observed that it has no vaue in the wound infection prophylaxy. We tried to convert the compoud fractures to the closed ones within ten days and have obtained very good results using the isoline method who have been using in the treatment of hand injuries, Besides this we administered intravenous antibiotics before performing debridman and thus provided the antibiotic level highest in blood and bones. This is very useful to reduce the infection rates. We aplied Rush pins, Rush rods and external fixators that can be easiy removed in some compound fractures. The results are advisible.
