Experience in Surface Replacement Arthroplasty (Wagner) in Osteoarthrosis of the Hip Joint

Abstract

The report describes 42 double cup endoprothesis of the hip (Wagner-cup) at an overage age 53,5 years. The procedure was mainly indicated in osteoarthrosis caused by dysplasia coxae, genuine osteoarthrosis of the hip joint. Joint surface substitution represents a very interesting surgical method in which, unlike in the case of total endoprothetic substitution the physiological state of the joint is preserved. However, technically, this is very difficult operation, which requires a great deal of skill. Most authors reporting of resutts published to date do not give sufficient information on late results, and the present publicalion is no exception. The author will be in a position to give an assessment only in a few years time. According to the authors experience, hip joint endoprothesis after Wagner is in many instances an enrichment of the alternative techniques available for dealing with coxarthrosis and it has shifted the position of total endoprothesis from the indication list.

WAGNER'İN KAPAK PROTEZİNİN KOKSARTROZ TEDAVİSİNDEKİ YERİ

Abstract

Bu çalışmada 1978 yılında yaptığımız yayına ek olarak 42 kapak protezi uygulamasının erken sonuçlarını bildirdik. Altmış yaşın altındaki koksartroz hastalarında son yıllarda gittikçe artan sayıda TEP, keramik protez ve kapak protezi takılmaktadır. Kapak protezi CHARNLEY'in ikinci savunma hattına olanak sağladığından, daha çok, hatta aşırı şekilde uygulanmaya başlamıştır. Son üç yıldır Orta Avrupa'da yaygın olarak yapılan bu ameliyatın başka kliniklerdeki sonuçlarım da topladık. Böylece önemli indikasyon hataları daha belirgin bir biçimde gösterilebilmiştir. Yaş ortalaması 53,5 yıl olan kollektifde bir defa femur boynu kırığı bir kere de santral luksasyon görüldü. Her ikisinde de TEP uygulandı. Başarısızlığa uğrayan kapak protezinin yerine hemen her olguda TEP takılabilmesi metodun en olumlu yanını teşkil etmektedir.
