During artroscopy of the knee appositons of the needle lateral patellar tendons

Abstract

The central transpatellar tendon arthroscopic approach and the standard lateral approach were compared in 200 unselected codaver knees using an arthroscopic dummy. In 108 knees transpatellar tendon approachment that the dummy passed through the infrapatellar fat pad. In 7 cases with a transpatellar and in 22 cases with a lateral approach we were unabte to reach the medial posterior compartment of the knee joint. It was usually easier to insert the orthroscopic dummy into the posterior fossa if the knee joint was held at a 35-40 of flexion of the knee than at 90 of flexion. In 88 of the 100 knees the dummy could be passed from the transpatellar tendon approach into the lateral posterior fossa and in 77 of the 100 knees from the lateral approoch. Patella baja was found in 16 knees (8%). Since the transpatellar tendon approach should not be used in these cases it means that this technique cannot be used in all cases. From an anatomical point of view the transpatellar tendon method thus does no offer any major advantages compared to the standard lateral one.

DİZ EKLEMİ ARTROSKOPİSİNDE PATELLA TENDONU ORTASI ULAŞIMI İLE PATELLA TENDONU LATERALİ ULAŞIMININ KARŞILAŞTIRILMASI (Kadavra Çalışması)

Öz

Patella tendonu ortasından geçen ulaşım ile alışılmış patella tendonu laterali ulaşımı, artroskop modeli kullanılarak rastgele 200 kadavra dizinde karşılaştırılmıştır. Patella ortası ulaşımında 198 dizde artroskop modeli patella altı yağ yastıkçığı içinden geçmiştir. Patella ortası ulaşımmda 7 dizde, lateral ulaşımda 22 dizde eklemin posterior medial bölümüne ulaşamadık. Artroskop modelinin dizin posterior fossasına sokulması, diz 35-40 derece fleksiyonda tutulduğu zaman daha kolay oluyordu. Dizin posterior loteral fossasına artroskop modelinin sokulması pafella tendonu ortası ulaşımında 100 dizden 88 inde. Patella tendonu laterali ulaşımında 100 dizden 77 sinde yapılabilmiştir. Patella baja 76 dizde bulunmuştur (%8). Patella baja olgularında patella tendonu ortası ulaşımının kullanılmaması gerektiğinden bu yöntemin bütün olgularda kullanılamayacagı anlamı çıkar. Bu nedenle ana;tomik görüş açısındm patella tendonu ortası ulaşımı yöntemi, alışılmış lateral ulaşım yöntemi ile karşılaştırıldığında büyük bir üstünlük göstermemektedir.
